Definition of terms

Throughout this website we use words and phrases that you might hear during advance care planning conversations. We have made a Glossary of Terms to explain what these words mean.

Language

We have tried to use the same words and phrases throughout this information resource.

We use the term advance care planning

This can also be called future or anticipatory care planning.

We use the term individual receiving care

This means the person receiving care and who the advance care planning discussion relates to.

We use the term those important to the individual

This might be a partner, relative, loved one, friend or caregiver who is involved in decision-making support for the individual receiving care.
